It seems that the US withdrawal of its draft resolution to expand the mandate of the MINURSO to monitor human rights in the Sahara was disillusioning and traumatic to the Polisario and Algeria. Following the adoption by the UN Security Council of the resolution on the Moroccan Sahara by consensus?the resolution that makes no mention of a human rights monitoring mechanism in the Moroccan Sahara?the separatists in the southern provinces went into riots in Laayoune and Boujdour to provoke the security forces and instigate violence.
The US withdrawal of its draft resolution, which is perceived as a political victory for Morocco, puts Algeria and Polisario in an embarrassing situation. In response to this political victory, Algeria and Polisario plan to sow the seeds of discord and anarchy in the Moroccan Sahara by ?inciting minors to engage in clashes with Moroccan security forces to promote the claim that Morocco does not respect human rights,? the mayor of Laayoune Khalil Eddakhil said in a press conference on Monday.
The mayor added that there is information indicating that a number of ?young men benefit from training in the Tindouf camps (of Polisario in the Algerian territory) and then infiltrate Morocco?s southern provinces to carry out acts of sabotage.?
The plan to drag the region into instability, of which Morocco is fully aware, is carried out by the separatists in the cities of the Moroccan Sahara. These individuals were not arrested despite the damage that they caused to public property in these cities. ?No one was arrested despite the acts of vandalism and serious damage that occurred to public property, as well as the injury of a number of members of the security forces who were pelted with stones,? says Mr. Khalil Eddakhil who also stressed that ?Morocco is committed to the policy of restraint so as to not give the opponents of its territorial integrity excuses that they later may use against it.?

Wikimedia Commons, which acts as Wikipedia's media repository, has pushed its Android and iOS app out of the beta phase, giving photo collecting for the digital encyclopedia a workflow that isn't chained to a browser. By registering an account online and wielding the software, users can upload photographs taken with the application -- or within their device's library -- straight to the organization's database, and endow them with titles and descriptions. The chairman of the House Homeland Security Committee said Sunday that federal authorities are investigating whether the suspects in the Boston marathon bombing received training that helped them carry out the deadly attack.
Rep. Michael McCaul (R-Texas) said during an appearance on Fox News Sunday that it was too soon to dismiss a possible connection between the suspects and foreign terrorists.
"Right out-of-the-box, U.S. officials unanimously are saying there's not foreign connection to this case when in fact the FBI just began its investigation into the case," McCaul said on Fox News Sunday. "They just got the computer. They just sent a U.S. team over to (the) Chechen region, and to Dagestan, to interview witnesses."
He added: "I think given the level of sophistication of this device, the fact that the pressure cooker is a signature device, goes back to Pakistan or Afghanistan, leads to believe ? and the way they handled these devices and the trade craft leads me to believe that there was a trainer. And the question is, where is that trainer or trainers? Are they overseas in the Chechen region or are they in the United States?"
Publicly, U.S. officials investigating the bombing said there is no evidence of a wider plot, including training, direction or funding for the attacks.
And on CBS' Sunday morning show, Face the Nation, Sen. Claire MCaskill (D-Missouri) said there was no evidence the suspects were "part of a larger organization."
Dzhokhar Tsarnaev, 19, is charged with joining his older brother, Tamerlan, who's now dead, in setting off the bombs near the marathon finish line. The attacks killed three and wounded 264. The brothers are ethnic Chechens who came to the United States about a decade ago with their parents. Both parents now live in Russia.
McCaul said Sunday that the suspects' mother had contributed to their "radicalization" and would be detained for questioning if she returned to the United States.
Meanwhile, Reuters reported Sunday that the parents of the bombing suspects had scrapped plans to the travel to the United States.
During an interview from an undisclosed location in Russian, the suspects' father, Anzor Tsarnaev, told the wire service that he believed he would not be allowed to see his surviving son Dzohkhar, who was captured and has been charged in connection with the April 15 bomb blasts that killed three people and wounded 264.
"I am not going back to the United States. For now I am here. I am ill," Tsarnaev said. "Unfortunately I can't help my child in any way. I am in touch with Dzhokhar's and my own lawyers. They told me they would let me know (what to do)," he said.
Tsarnaev had said Thursday that he planned to travel to the United States to see Dzkhokhar and bury his elder son, Tamerlan, who was shot dead by police in a firefight four days after the bombings.
Reuters said Tsarnaev agreed to the face-to-face interview on condition that the village's location not be disclosed.

Kosovo court jails 3 in organ trafficking case
PRISTINA, Kosovo (AP) ? A Kosovo court has found two ethnic Albanians guilty of human trafficking and organized crime in a highly publicized trial against seven people suspected of running an international organ trafficking ring.
A panel of two European Union and one Kosovo judges sentenced Lutfi Dervishi to eight years in prison and his son Arban Dervishi to seven years and three months in prison on Monday for extracting kidneys from poor donors who were lured by financial promises.
A third defendant was jailed for 3 years and another two released on parole for causing grievous bodily harm. Two defendants were acquitted of fraud and abuse of authority. The defendants can appeal the verdicts.
The EU prosecutes sensitive cases since Kosovo's contested 2008 secession from Serbia.

Thunder take 3-0 series lead with 104-101 win

HOUSTON (AP) ? Kevin Durant scored 41 points in his first game without injured All-Star Russell Westbrook, and the Oklahoma City Thunder fought off Houston's late rally to beat the Rockets 104-101 on Saturday night for a 3-0 lead in the best-of-seven playoff series.
Durant scored 27 points in the first half, and the Thunder led by 26 points before a big third-quarter rally by the Rockets put them within striking distance.
Houston regained the lead with about 4 minutes left, and was up with less than a minute remaining. But Durant hit a 3-pointer seconds later to put Oklahoma City on top 100-99.
A turnover by James Harden gave the Thunder the ball back and Derek Fisher made two free throws to push the lead to 3. Harden made a layup, but Reggie Jackson hit two more free throws after that.
Houston had a last chance, but Carlos Delfino's 3-pointer fell short.
Harden scored 30 points to lead the Rockets in the first playoff game in Houston since 2009.
Game 4 is Monday night in Houston
Westbrook was injured in Game 2 and had season-ending knee surgery Saturday. It was the first time Durant had ever played without Westbrook, who hadn't missed a game in his career before Saturday.
Durant tied a career high for the playoffs, and he added 14 rebounds, four assists, two steals and a block. Jackson started in place of Westbrook and had 14 points, and Ibaka added 17 points and 11 rebounds.
A 3-pointer by Chandler Parsons tied it with 5:46 left and Delfino gave Houston its first lead since early in the first quarter, 94-93, about 2 minutes later.
Ibaka made four straight points after that, ending with a reverse layup, to make it 97-94 with about a minute and a half left.
Harden hit a pair of free throws. Then Francisco Garcia's 3-pointer put them up 99-97 with 45 seconds left, before the clutch shot by Durant.
The Rockets rallied from a 15-point deficit to take a lead in the fourth quarter in Game 2. But they weren't able to hold on and Oklahoma City got the 105-102 win.
It was much the same this time, as Houston went up late only to fall again.
Westbrook's absence didn't seem to bother Oklahoma City early, as Durant had the big first half and the Thunder looked to be in complete control.
Things changed in the third quarter, as Durant was limited offensively and Houston outscored the Thunder 27-14 in the quarter.
Houston point guard Jeremy Lin, who suffered a bruised chest muscle in Game 2, started but scored only two points in about 18 minutes.
Parsons added 21 points for Houston, and Garcia had 18.
After the big run got Houston within eight points in the third quarter, Garcia and Harden made 3-pointers around a dunk by Durant to whittle the lead to 80-76 entering the last quarter.
Durant was scoreless in the third quarter until his dunk with 27 seconds remaining in the quarter.
The Thunder led by 19 points when the Rockets used a 16-5 run to get to 78-70 with 2 1/2 minutes left in the third quarter. The Rockets harassed Oklahoma City into three turnovers in that stretch, and Durant went cold as he missed four shots.

Twenty adopted Russian children have died at the hands of their American parents.
This is the justification Russia gives for banning U.S. adoption of Russian-born children.
Russia?s commissioner for human rights, democracy and rule of law, Konstantin Dolgov, met with U.S. officials April 17 and 18 to discuss international adoption policy according to Turkish Weekly.
U.S. congressional sources say Russia refuses to complete adoptions that were in progress before the ban on Jan. 1.
?I feel so sorry for the families and the children this (law) affects,? said Tim Rouse, projects manager for the administration division at Guilford College, who adopted newborn Cassidy Jane from Florida this year.
?I can tell you from experience that, when you lose a placement that you really believed was going to happen, it can be devastating,? said Rouse. ?For some, it is just as painful as having a miscarriage. It is heartbreaking. A better solution has to be out there.?
Russian President Vladimir Putin signed the ban in December in retaliation of the U.S. Magnitsky Act passed late last year, named after a Moscow lawyer who died in a Russian prison, according to National Journal. This law denies visas to Russians accused of human rights violations and freezes their assets in the U.S.
At least 700 American families have filed for adoption with the Russian government and are now waiting for the ban to end according to Jan Wondra, vice chair for Families for Russian and Ukrainian Adoption.
Of these families, an estimated 230 have already visited their prospective children and now worry whether they ever will again.
Time and money are another flaw in the long process of adoption.
?There?s a lot of money that changes hands in going to adopt,? said Christine Lawe who adopted her daughter Anya from China in an interview with the Guilfordian.
?Is it going directly to support that orphanage? Are people?s palms being greased?? said Lawe. ?You do not know where your money is going to so there is something uncomfortable about that.?
Over 600,000 Russian children currently live outside the custody of their biological parents according to The New York Times. Although most live in foster homes, more than 130,000 live in Russia?s orphanage center, including many with mental and physical health problems.
?If you are going to adopt, you should adopt from wherever it?s needed most,? said Early College student Thomas Lawe, Anya?s brother. ?(Adopt from a country) where orphanages are not a good place to be.?
After the ban, filmmaker Olga Loginova for Radio Free Europe visited her biological daughter?s adopted mother, Tina Traster, to make a documentary proving that successful Russian adoptions do exist, according to The Daily Beast.
?No one denies that 20 deaths of Russian-adopted children is a disturbing statistic,? wrote Traster in her Women in the World article for the Daily Beast. ?Something is not right ? I know this because I adopted a child from Russia and because I speak to other adoptive parents of Russian children all the time.?
Less than two months after this law took effect, three-year-old Mad Shatto, adopted from Russia, was found dead outside his Texas home.
Supporters of the ban claim Shatto was murdered by his adoptive mother, though Texas investigators determined his death was a self-inflicted accident, according to the New York Times.
Regardless, in the eyes of Russian President Putin, this is yet another case of an adopted Russian child dying on U.S. soil.
In the eyes of Russian foster children, however, it is slamming the door shut on the home and family they almost had.
?To me, adopted children are more than loved, they are chosen,? said Rouse. ?The only other person that we get to choose to be a part of our family is our spouse. And to me, that makes the relationship all the more special.?

Obama jokes about radical 2nd term changes
WASHINGTON (AP) ? President Barack Obama joked Saturday about his plans for a radical second-term evolution from "strapping young Muslim Socialist" to retiree golfer, all with a new hairstyle like first lady Michelle's.
Obama used this year's annual White House Correspondents' Association dinner to poke fun at himself and some of his political adversaries, asking if it was still possible to be brought down a peg after 4? years as commander-in-chief.
Entering to the rap track "All I Do Is Win" by DJ Khaled, Obama joked about how re-election would allow him to unleash a radical agenda. But then he showed a picture of himself golfing on a mock magazine cover of "Senior Leisure."
"I'm not the strapping young Muslim Socialist that I used to be," the president remarked, and then recounted his recent 2-for-22 basketball shooting performance at the White House Easter Egg hunt.
But Obama's most dramatic shift for the next four years appeared to be aesthetic. He presented a montage of shots featuring him with bangs similar to those sometimes sported by his wife.
Obama closed by noting the nation's recent tragedies in Massachusetts and Texas, praising Americans of all stripes from first responders to local journalists for serving the public good.
Saturday night's banquet not far from the White House attracted the usual assortment of stars from Hollywood and beyond. Actors Kevin Spacey, Julia Louis-Dreyfus and Claire Danes, who play government characters on series, were among the attendees, as was Korean entertainer Psy. Several Cabinet members, governors and members of Congress were present.
And despite coming at a somber time, nearly two weeks after the deadly Boston Marathon bombing and 10 days after a devastating fertilizer plant explosion in West, Texas, the president and political allies and rivals alike took the opportunity to enjoy some humor. Late-night talk-show host Conan O'Brien headlined the event.
Some of Obama's jokes came at his Republican rivals' expense. He asked that the GOP's minority outreach begin with him as a "trial run" and said he'd take his recent charm offensive with Republicans on the road, including to a book-burning event with Rep. Michele Bachmann.
Casino magnate Sheldon Adelson would have had better success getting Obama out of office if he simply offered the president $100 million to drop out of last year's race, Obama quipped.
And on the 2016 election, the president noted in self-referential irony that potential Republican candidate Sen. Marco Rubio wasn't qualified because he hasn't even served a full term in the Senate. Obama served less than four years of his six-year Senate term before he was elected president in 2008.
The gala also was an opportunity for six journalists, including Associated Press White House Correspondent Julie Pace, to be honored for their coverage of the presidency and national issues.
The New Yorker's Ryan Lizza won the Aldo Beckman Award, which recognizes excellence in the coverage of the presidency.
Pace won the Merriman Smith Award for a print journalist for coverage on deadline.
ABC's Terry Moran was the winner of the broadcast Merriman Smith Award for deadline reporting.
Reporters Jim Morris, Chris Hamby and Ronnie Greene of the Center for Public Integrity won the Edgar A. Poe Award for coverage of issues of national significance.

Analysis: In force-feeding detainees, Obama has courts on his side

WASHINGTON/MIAMI (Reuters) - As detainees at the U.S. military prison in Guantanamo Bay, Cuba, press ahead with a widening hunger strike nearly three months old, President Barack Obama has come under increasing criticism for his policy of force-feeding them.
But U.S. law is on his side, an analysis of court rulings shows.
Most U.S. judges who have examined forced feeding in prisons have concluded that the measure may violate the rights of inmates to control their own bodies and to privacy - rights rooted in the U.S. Constitution and in common law. But they have found that the needs of operating a prison are more important.
Courts generally view a prison hunger strike as a suicide attempt, and they have ruled wardens have authority to stop suicide attempts as part of their mandate to preserve order.
"If prisoners were allowed to kill themselves, prisons would find it even more difficult than they do to maintain discipline, because of the effect of a suicide in agitating the other prisoners," Judge Richard Posner wrote for a Chicago-based appeals court in 2006 in a case involving a Wisconsin prison that punished a disobedient inmate by refusing him food.
As of Thursday, 94 of the 166 prisoners were on a hunger strike in Guantanamo, meaning they had refused at least nine consecutive meals. According to a military count, 17 had lost enough weight to be force-fed liquid meals through a nasogastric tube, and three were in the hospital for observation.
Army Lieutenant Colonel Samuel House, a spokesman for the detention camp, said none of the detainees in the hospital had a life-threatening condition.
Striking inmates began refusing to eat around early February, alleging rough handling of the Koran during searches for contraband and protesting their prolonged imprisonment. General John Kelly, head of U.S. military forces in Latin America, said assertions about the Koran were untrue.
OPPOSITION VOICED
A New York Times opinion piece last week by Samir Naji al Hasan Moqbel, a Yemeni man detained at Guantanamo since 2002, launched debate over the forced feedings. Like others there, he was captured abroad on suspicion of supporting terrorism.
"I will never forget the first time they passed the feeding tube up my nose. I can't describe how painful it is to be force-fed this way," Moqbel said in the op-ed dictated through an interpreter to his lawyers.
As described by Guantanamo officials, a feeding tube is lubricated and inserted through the nose down to the stomach for the two hours it takes liquid food to pass through. In general, hunger strikers continue to drink water.
Human rights advocates and many doctors decry forced feeding of hunger strikers as a violation of personal liberty and medical ethics with risks of medical complications such as discomfort, bleeding, nausea and throat sores. The 65-year-old World Medical Association, made up of 100 national medical associations, has said it is unethical and never justified to force-feed a mentally competent adult.
Carlos Warner, a federal public defender who represents 11 Guantanamo detainees, including Kuwaiti hunger striker Faiz al Kandari, said detainee lawyers are split on the issue.
Some "have a clear position that the government should not be force-feeding," and have unsuccessfully made their argument in federal court in Washington, D.C., Warner said. "Other lawyers are of the opinion that their clients should not die of hunger before we have a chance to free them."
The Constitution Project, a U.S. legal group that includes Democrats and Republicans, said last week that forced feeding at Guantanamo "is a form of abuse and must end."
George Annas, a Boston University professor of health law who opposes the forced feeding of hunger strikers on medical ethics grounds, said U.S. law is "very permissive" of the practice. He described the attitude of American prisons as: "Do we care about indignity? No, you're a prisoner, we'll treat you the way we want."
BARRIERS TO LEGAL CHALLENGE
The U.S. military argues forced feeding is not only legal but also humane. A federal judge agreed in 2009, ruling against Mohammed Bawazir, a Yemeni national held at Guantanamo since 2002. Bawazir called forced feeding torture.
Bawazir cited pain he experienced and use of a chair with "six-point restraints" that kept in place his forehead, limbs and torso. U.S. District Judge Gladys Kessler in Washington, D.C., said officials acted out of a need to preserve life.
A further barrier to any suit is the Military Commissions Act of 2006 which bars U.S. courts from hearing cases about Guantanamo detainee treatment. Even if they were to hear a challenge to forced feeding, the overriding evidence is the courts would rule against the detainees.
International law, which prohibits the inhuman or degrading treatment of prisoners, is not necessarily any help to Guantanamo detainees either.
The European Court of Human Rights has ruled at least twice that forced feeding amounted to torture, in a 2005 case out of Ukraine and in a 2007 case out of Moldova, but it stopped short of barring the procedure. The court said it may be used to preserve the life of hunger strikers if shown to be medically necessary and not done for punitive reasons.
Other reports of non-U.S. countries using forced feeding are rare, although experts said there is a lack of data. The practice has been described in news reports in Bahrain, China and Greece during the past decade.
Ten British-held Irish Republican Army prisoners, including former IRA commander Bobby Sands, starved to death in the Maze prison in Northern Ireland during a 1981 hunger strike during which they sought recognition as political prisoners. It ended when the families of the remaining hunger strikers authorized doctors to begin life-saving intravenous feeding as soon as the prisoners lost consciousness.
One early U.S. case involved Mark David Chapman, convicted in the 1980 killing of former Beatle John Lennon. Chapman broke a 26-day fast in 1982 only under a New York court-ordered threat he would be force-fed. Now 57, he is serving a prison sentence of 20 years to life.
Chapman had said he wanted to draw attention to starving children, but the court ruled the state's obligations to protect life and maintain order in its institutions outweighed Chapman's rights to free expression and to privacy.
BALANCING TEST
Most federal and state courts have agreed.
In Rhode Island, for example, finding that prisons have a duty to protect inmates' health, the State Supreme Court ruled in 1995 that "it would be in total disregard of this duty to stand idly by while a healthy adult decided to end his or her life by starvation just as it would if he or she decided to end his or her life by some more dramatic means such as hanging, slashing of wrists, or swallowing some type of poison."
Courts in three states - California, Florida and Georgia - have been exceptions, restricting forced feeding on various grounds, Mara Silver wrote in a 2005 Stanford Law Review article on the constitutional question of self-starvation.
California was the most sweeping. Prison officials must demonstrate "a threat to institutional security or public safety," not merely the conjecture of one, before denying an inmate the choice not to eat, the state's high court ruled.
Those are the exceptions however.
"The federal courts analyze the issue by balancing the prisoner's right to autonomy against the prison's right to maintain order, and they nearly always find that force-feeding is constitutional," said Margo Schlanger, a University of Michigan law professor with expertise in prisons.

BOSTON (AP) ? The New York Knicks want to finish their long road back to the second round quickly.
They haven't won a playoff series in 13 years, and now they have a shot to end this one in four games.
"Guys are hungry," forward Carmelo Anthony said. "We really want this, we feel like it's right there in the palm of our hands."
Anthony scored 26 points and the Knicks moved to the brink of a sweep, routing the Boston Celtics 90-76 on Friday night in Game 3 of the first-round series.
Quickly taking the crowd out of it in the first NBA game here since the Boston Marathon bombings, the Knicks built a 16-point halftime lead and now are on the verge of taking out the Celtics.
"That first quarter on the road, you've got to make sure you take that first punch and we did that," Knicks veteran Jason Kidd said.
Raymond Felton added 15 points and 10 assists for the Knicks, who haven't advanced in the playoffs since reaching the 2000 Eastern Conference finals. Game 4 is here Sunday afternoon.
They'll have to hope they have J.R. Smith, who inexplicably threw an elbow right into Jason Terry's face with 7:06 left and was thrown out of the game with a flagrant foul 2, an automatic ejection. The league office will review the play and can fine or suspend Smith.
Jeff Green scored 21 points for the Celtics, who will try to become the first NBA team to win a series after trailing 3-0. They own the most titles in league history, but their present looks extremely bleak.
"I didn't think we showed up not to play well," Celtics coach Doc Rivers said. "I thought overall our effort was there, I thought we did lose our spirit early on."
Fans cheered the responders who helped after the marathon but booed the Celtics, who managed only 31 points in the first two quarters, getting their bad half out of the way early this time. They had been held to 25 and 23 points after halftime while dropping the two games in New York.
Smith finished with 15 points for the Knicks, who are loaded with veterans who've had playoff success the franchise hadn't in recent years and weren't fazed by the pregame atmosphere. Once the game started, they simply outplayed the Celtics, as they have while going 6-1 against them dating the regular season.
"We came in here believing that we could win this basketball game and we did it," Anthony said. "It was a total team effort."
Everything went perfectly for the Knicks until Smith, while holding the ball on offense with a huge lead, decided to throw his elbow right into Terry's face. Terry charged after him but was stopped by Rivers, while Knicks coach Mike Woodson appeared to be lecturing Smith before the Sixth Man of the Year was sent off.
"He'll learn from it. I don't think he was trying to hurt the kid, I thought he was trying to clear space, but hey, they saw it differently," Woodson said. "I'm going to stay in his ear and make sure he learns from it."
There was a moment of silence before the game for the victims, and that was the last time it was quiet for a while. First responders and other personnel were greeted with loud cheers as they were brought onto the floor during timeouts, and a tribute to the city's strength played on the overhead video board during a first-quarter stoppage.
But the Knicks' dominance wore down even the most spirited supporters, just the way LeBron James did in his 45-point performance in Game 6 of last year's Eastern Conference finals in the last playoff game here.
Rivers said he was aware of some of his team's shortcomings, with the absence of Rajon Rondo leaving the Celtics without a point guard. But Rivers ? himself a former point guard ? couldn't have imagined his team would be this unable to get into an offense, finishing with 18 turnovers that led to 26 Knicks points.
He started Terry for forward Brandon Bass, trying to find a way to get another ball handler on the floor. But it couldn't fix the Celtics' shooting and didn't really help much with the ball handling, as Terry had his dribble taken right away from him on one third-quarter possession when Boston briefly looked ready to get back into it.
Paul Pierce and Kevin Garnett missed layups on the same possession to start the game, and it was clear right from there this wasn't the Celtics' night ? or their series.
"Right now at this point, just take it one game at a time," Green said. "They haven't won it yet."
Pablo Prigioni gave the Knicks a boost with three early 3-pointers as the Knicks positioned themselves to avenge their first-round sweep at the hands of the Celtics two years ago. Boston hasn't been by swept since 2004, by Indiana.
Garnett had 12 points and 17 rebounds, while Pierce missed six of his first seven shots and finished with 17 points.
The Celtics were within three early in the second quarter before Prigioni and Smith hit 3-pointers in an 8-0 run that made it 34-23. The Celtics had held Anthony in check until then, but he got hot late in the half while they remained ice cold.
Back-to-back jumpers had the defense keying on his jumper, so he spun toward the basket to catch an alley-oop pass, and Felton followed with a 3-pointer to make it 47-29 with 58 seconds left in the half. Green made a layup for the final basket of the half, then the Celtics walked off to boos.
Boston cut it to 49-38 and Terry was dribbling up the floor on a fast break when Prigioni stepped in and swiped the ball away from him. The Knicks missed a shot, but Prigioni grabbed the rebound to set up a 3-pointer by Anthony. Felton followed with another basket to make it 54-38, and Boston's momentum was gone for good.
The Knicks later led by as much as 21.
Boston has been held to 78, 71 and 76 points in the series.
Notes: The Knicks' last sweep of a best-of-seven series was when they beat Atlanta in the 1999 East semis. They swept Toronto in a best-of-five series in the first round the following year. ... Rivers, on being fined $25,000 by the NBA on Thursday for his criticisms of the officiating in Game 2. Examine social factors to explain rise in diagnoses of mental disorders
Apr. 25, 2013 ? Examining social factors is vital to better explaining and understanding the dramatic rise in the number of Americans diagnosed with mental disorders in recent years, according to an analysis by a team of medical and mental health experts.
Their conclusions, which appear in the latest issue of the journal Health Affairs, comes ahead of the May release of the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (DSM-5), a comprehensive guide that sets the classification, diagnosis, and treatment of mental disorders across the United States and the world.
The study included researchers from New York University, Columbia University, the University of California, Berkeley, and Rutgers University.
In their analysis and commentary, the authors argue that the forthcoming DSM-5, which is used by all psychiatrists, psychologists, and mental health workers in the U.S., has missed crucial population-level and social determinants of mental health disorders and their diagnosis. As a result, the DSM may be mischaracterizing the rates of certain afflictions.
"If we are to believe current reports, there are 12 times more children with Attention Deficit Hyperactivity Disorder (ADHD) in the U.S. than in Europe, and within the U.S., there are almost 50 percent more children with ADHD today than a decade ago, according to DSM," observes the article's lead author, Helena Hansen, MD, PhD, an assistant professor of anthropology at NYU and an assistant professor of psychiatry at NYU Langone Medical Center. "My colleagues and I wanted to know if there was something else behind this."
"To explore this, we assembled a group of population health experts to identify the best way to explain the rise in these diagnoses. And what we found was that the clinical authorities in psychiatry who revise the DSM are unable to take into account other forces that drive the diagnosis of mental disorders."
To address this matter, the researchers posed three possible causes of the rise in diagnoses that are not currently accounted for in revisions of the DSM:
* Is there a change in the environment causing an actual increase in the mental health problem? For example, have the pressures of standardized testing in the U.S. caused ADHD symptoms?
* Are the diagnostic criteria applied differently depending on the institutional and social environment? For instance, do the increasing numbers of children diagnosed with ADHD reflect pharmaceutical company promotion of ADHD awareness and ADHD medications among school teachers and parents? Among low income children, do diagnoses reflect their effort to qualify for disability benefits in the wake of welfare reform?
* Are the diagnostic criteria written in a way that includes people who do not have a disorder? For example, do the criteria for ADHD of excessive running, climbing, and talkativeness describe a high level of energy that should be expected among children?
The researchers also propose an independent review of these factors.
"To sort out which these three factors causes differences in the diagnosis of a mental disorder over time and place, we need a review body that acts independently of the authors of the DSM and that is composed of experts on population health and the social factors driving mental health," says Hansen. "Using the example of ADHD, the review body might look at the number of children diagnosed under different educational and welfare policies -- and before and after pharmaceutical promotions. It might also examine how changes to the criteria for ADHD in each revision of the DSM affect the number of children given the diagnosis."
"By charging experts with independent review of the best available research on population and social variation in the diagnosis of mental disorders, we can identify unconsidered but powerful causes of diagnosis and inform future revisions of the DSM."
The analysis, which was supported by the Robert Wood Johnson Health and Society Scholars Program, will appear in the May issue of Health Affairs.

Starbucks second-quarter profit rises
(Reuters) - Starbucks Corp
The world's biggest coffee chain reported net earnings of $390.4 million, or 51 cents per share, for the fiscal second quarter that ended on March 31. That compares with $309.9 million, or 40 cents per share, a year earlier.
Revenue rose 11 percent to $3.56 billion.

P&G, AT&T drop after earnings, holding back Dow
NEW YORK (AP) ? Weak earnings forecasts from Procter & Gamble and AT&T held the Dow Jones industrial average back on Wednesday.
P&G, the world's largest consumer goods maker, fell 4.8 percent to $78.03 after the maker of Tide detergent and Gillette razors said its revenue fell short of the expectations of Wall Street analysts. The company also issued a weak profit forecast for the next quarter. AT&T dropped 5.9 percent to $36.70 after it lost phone subscribers from its contract-based plans for the first time as sales of smartphones slow.
Those declines were enough to keep the 30-member Dow in the red, despite more stocks rising than falling in the index. The Dow was down 24 points by noon Eastern time.
In the broader Standard & Poor's 500 index, the earnings news was mixed.
Yum Brands, which owns KFC, Pizza Hut and Taco Bell, was among the gainers, advancing 7 percent to $68.70. Yum reported earnings late Tuesday that exceeded the expectations of financial analysts. General Dynamic, the aerospace and defense company, also surged after posting a profit that was better than expected. The stock jumped 5.3 percent to $70.66.
Other companies, including home appliance maker Whirlpool and drug maker Eli Lilly, disappointed investors.
The Dow Jones industrial average was down 0.2 percent at 14,695. The S&P 500 index rose one point, or 0.1 percent, to 1,579. The Nasdaq composite was down six points, or 0.2 percent, at 3,264.
While the majority of companies have been exceeding Wall Street's expectations on earnings, their performance on sales hasn't been as strong, suggesting that they are struggling to grow.
About 67 percent of S&P 500 companies that have reported earnings so far have beaten expectations, better than the 10-year average of 62 percent, according to S&P Capital IQ. However, only 42 percent have beaten revenue forecasts.
Company earnings so far have been consistent with signs that the global economy is slowing, said Jim Russell, a regional investment director at U.S. Bank. Russell said it's too early to conclude whether the slowdown will be short-lived.
"We think that most managements are appropriately cautious in their outlooks, because it's very possible that the second-quarter will continue to slow," said Russell. "We're watching with cautious optimism that this is a second-quarter-only soft patch in the economic data."
A report Wednesday that orders for long-lasting U.S. factory goods fell more than economists expected last month added to the signs that global growth is cooling. The Commerce Department said orders for durable goods declined 5.7 percent in March following a 4.3 percent gain the previous month. February's figure was also revised lower.
Stocks logged their biggest weekly drop in five months last week after growth in China, the world's second-biggest economy, slowed. Weaker hiring and manufacturing growth in March have also weighed on the stock market this month.
The market's gains this month have been modest after a first-quarter surge that pushed both the Dow Jones and the S&P 500 to record highs. The Dow is up just 0.8 percent in April, the S&P 500 index 0.6 percent.
During the first three months of the year, the Dow and the S&P 500 averaged monthly gains of more than 3 percent, driven by optimism that the housing market was recovering and company earnings would continue to climb.
Among other companies that reported earnings, Boeing rose 3.6 percent to $91.27 after the airplane maker said its first-quarter net income rose 20 percent despite problems with the 787 Dreamliner. The company said it would still meet its financial and delivery targets this year even after the 787 was grounded in mid-January because of problems with its batteries.
Amgen plunged 6 percent to $105.80 after the biotech company's sales growth disappointed investors.
In government bond trading, the yield on the 10-year Treasury note, which moves inversely to its price, was little changed at 1.71 percent.
